The Partners Against Pain^® Pain Management Kit contains a diverse selection of forms and other assessment tools that can be used to record pain intensity and duration, patient consent and treatment, clinical follow-up, and patient response to pain treatment.

The kit also provides several styles of pain diaries for use by your patients, drug abuse screening tools for your use, as well as important reference papers that document the latest consensus on the treatment of chronic pain, the use of opioids, and authoritative definitions related to using opioids for treating pain.

Select the forms and assessment scales you prefer, duplicate them for clinical use, and integrate them into your clinical practice to enhance your clinical records and to document your ongoing interaction with patients.
